You have two options to insert the balloons, horizontal and vertical. My idea is to add a third option, which is going to insert the balloons into the horizontal but grouped in the vertical (today it groups the balloons only in the horizontal).
Automatic Grouping can become a complex task as detailed user requirements vary. Here are some of the items that would need to be considered for Automatic Balloon grouping: number of rows, number of columns, column - row direction, and sort order to name a few.
Good news is Balloons can easily be rearranged (vertically & horizontally) after placement using move function. Below are common approaches used.
Use Element Selection Tool to select desired Balloons to be moved > Use Move Element function to move
Use Drag Selection (select desired Balloon and hold left mouse button) > While holding left mouse button, move Balloon to desired location.
